TCM Herbal Treatment for Pneumonia

(1) Exterior Pathogen Invading Lung and Defensive Qi: Treatment Principle: Dispel wind, relieve exterior symptoms, promote lung function, resolve phlegm.
Formula: San Ao Tang or Sang Ju Yin with modifications. San Ao Tang is used for external invasion by wind-cold. Herbs include Ephedra, Apricot Kernel, and Licorice to disperse cold and promote lung function. If cold transforms into heat, presenting with fever, sweating, cough, and yellow phlegm, Ma Xing Shi Gan Tang is used to relieve exterior symptoms, clear heat, and stop cough. Sang Ju Yin treats external invasion by wind-heat. Herbs include Mulberry Leaf, Chrysanthemum, Mint, and Honeysuckle to cool and release exterior symptoms. Platycodon, Apricot Kernel, Licorice, and Reed Rhizome promote lung function, stop cough, and clear heat while generating fluids. Add Scutellaria baicalensis and Houttuynia cordata for excessive internal heat. For thirst and dry throat, add Adenophora stricta and Trichosanthes root to clear heat and generate fluids.
(2) Phlegm-Heat Blocking the Lungs: Treatment Principle: Clear heat, resolve phlegm, stop cough.
Formula: Ma Xing Shi Gan Tang combined with Wei Jing Tang (from *Qianjin Fang*) with modifications. Ephedra and Apricot Kernel promote lung function and resolve phlegm. Combined with Gypsum, they clear lung heat. Reed Rhizome, Coix Seed, Apricot Kernel, Peach Kernel, and Winter Melon Seed clear heat, resolve phlegm, and detoxify. For severe phlegm-heat, add Houttuynia cordata, Trichosanthes fruit, and Scutellaria baicalensis to enhance clearing and detoxifying effects.
(3) Heat-Toxin Invading Interior: Treatment Principle: Clear heat and toxins, clear heart, open orifices.
Formula: Qing Ying Tang with modifications. Rhinoceros Horn, Raw Rehmannia, and Salvia miltiorrhiza clear camp-level heat and cool blood. Combined with Bamboo Leaves, Coptis, Honeysuckle, and Forsythia, they clear heat and toxins, allowing camp-level pathogens to transfer to qi level for resolution. Ophiopogon and Scrophularia nourish yin and clear heat. If restlessness or delirium occurs, add Zi Xue Dan to enhance clearing heat and calming wind effects.
(4) Deficiency of Vital Qi with Persistent Pathogen: Treatment Principle: Nourish yin, clear heat, moisten lungs, resolve phlegm.
Formula: Zhu Ye Shi Gao Tang with modifications. Pinellia, Gypsum, and Bamboo Leaves clear residual heat and resolve phlegm to stop cough. Ginseng, Ophiopogon, Licorice, and Rice tonify qi and nourish yin. Add Adenophora, Raw Rehmannia, and Cortex Lycii to enhance nourishing yin and clearing deficient heat. Add Apricot Kernel, Loquat Leaf, and Trichosanthes peel to strengthen phlegm-resolving and cough-suppressing actions.
(5) Severe Deficiency Leading to Collapse: Treatment Principle: Restore yang, rescue from collapse, tonify qi and nourish yin.
Formula: Shen Fu Tang combined with Sheng Mai San. Ginseng and Aconite Root tonify qi, restore yang, and rescue from collapse. Ginseng, Ophiopogon, and Schisandra berry tonify qi and nourish yin, collectively achieving the effect of restoring yang and consolidating essence.
Inhalation Therapy: Using nebulizers to spray Chinese herbal solutions into the airways for treatment. Suitable options include Houttuynia cordata injection, Ma Xing Shi Gan Tang nebulization solution.
Guāyú Therapy: Apply hard coins dipped in plant oil or alcohol to scrape along both sides of the spine and scapular region until skin becomes congested. Used for fever and altered consciousness.
Enema Therapy: 10g each of Ephedra and Anemarrhena, 50g Gypsum, 10g each of Apricot Kernel and Licorice. Boil herbs, cool to 30°C, administer enema at 40ml per dose, 2–4 times daily. Suitable for severe pneumonia in children with intense heat.
