Adverse Reactions and Prevention of Shuang Huang Lian Injection

The main components of Shuang Huang Lian Injection are honeysuckle (Jinyinhua), skullcap (Huangqin), and forsythia (Lianqiao), possessing strong antibacterial and antiviral effects. It is clinically used for treating acute upper respiratory tract infections, herpangina, acute laryngitis, acute tonsillitis, acute bronchitis, and pneumonia. Administration involves intravenous infusion in glucose solution.
In recent years, the clinical use of Shuang Huang Lian Injection has become widespread, and reports of adverse reactions have increased annually. Observed adverse reactions include rash, diarrhea, abdominal pain, vomiting, skin itching, dizziness, chest tightness, and occasionally anaphylactic shock. Statistics indicate that rash is the most common adverse reaction, mostly appearing on the face, presenting as single or multiple maculopapular rashes; some cases show generalized urticaria, and very rarely, erythematous papules. Next, abdominal pain and diarrhea are less common than rashes. This suggests that the primary adverse reactions of Shuang Huang Lian Injection are likely due to its large molecular weight, predisposing it to allergic reactions, though the exact mechanism remains unclear.
Since rash-related allergic reactions are the most common adverse events, and most occur within half an hour or 3–5 days after initiation, close monitoring for rashes during the initial days of treatment is essential. If a rash appears, medication should be stopped immediately. Most rashes resolve on the same day. If accompanied by abdominal pain, diarrhea, or severe itching, injection of promethazine or intravenous dexamethasone should be administered after stopping the drug. In cases of anaphylactic shock, immediate cessation of medication is required, followed by oxygen administration, intravenous dexamethasone, and intramuscular epinephrine. Shock symptoms typically improve rapidly, with a favorable prognosis.
To prevent adverse reactions from Shuang Huang Lian Injection, careful observation during intravenous infusion is necessary. Using the same batch number throughout several days of treatment can reduce the incidence of adverse reactions. Additionally, it is best to administer Shuang Huang Lian Injection separately, avoiding mixing it with other antibiotics in the same infusion bottle, which may also reduce adverse reactions. Patients with a history of allergy or rash should not use Shuang Huang Lian Injection again.
