2. Treatment Methods:
① Bile-Expelling Formula Composition and Modifications: Jin Qian Cao 30g, Yin Chen 30g, Hu Zhang 9g, Yu Jin 9g, Pu Gong Ying 15g, E Zhu 15g, Chi Shao 9g, Wu Mei 6g, Chai Hu 10g, Ji Nei Jin 10g. During acute phase with liver-gallbladder heat stagnation, add Jin Yin Hua 10g, Huang Qin 9g; for obvious stagnation, add Mu Xiang 6g, Chuan Shan Jia 9g; for severe pain, add Yan Hu Suo 9g, Mu Gua 9g. During remission, for liver depression and spleen deficiency, add Qing Pi 9g, Bai Zhu 10g. For damp obstruction in the spleen-stomach, add Ping Wei San; for marked qi deficiency, add Huang Qi 10g.
② Acupuncture Points and Combinations: Ri Yue, Yang Ling Quan, Qiu Xu, Tai Chong, Gallbladder Point. For prominent rib pain and vomiting, add Qi Men; for severe abdominal distension and constipation, add Zu San Li, Jie Xi. Use draining technique in acupuncture.
3. Treatment Outcomes: We observed 40 patients; 17 passed stones, total effective rate 87.5%.
4. Discussion: Cholelithiasis complicated by cholecystitis falls under TCM categories such as "rib pain" and "accumulation." Given that the condition often arises from overindulgence in rich foods or overwork, closely related to damp-heat, qi stagnation, and blood stasis, we developed the bile-expelling formula. Ingredients like Jin Qian Cao, Yin Chen, Chi Shao, and Ji Nei Jin effectively clear heat and dampness, dispel stasis, relieve pain, and promote stone expulsion—well aligned with the disease mechanism. Acupuncture selects points such as Ri Yue, Yang Ling Quan, Qiu Xu, and Tai Chong, using draining techniques to "unblock meridians" and facilitate gallstone expulsion. Research confirms that needling Qiu Xu enhances regular gallbladder contraction and motility; Tai Chong relaxes Oddi sphincter spasm; Yang Ling Quan enhances gallbladder contraction and emptying capacity. Thus, combining acupuncture with herbal medicine achieves better therapeutic outcomes.
