In the ancient text "Discussions on the Supreme Dao of the World" from the pre-Qin period, "leakage" (xie) refers precisely to this condition. The "Chen's Book of Respectful Life" describes it as "ejaculation before intercourse or immediately upon penetration." The causes of premature ejaculation are mostly psychological, with organic lesions being rare. Therefore, treatment should focus first on re-establishing reflexive ejaculatory control, while eliminating potential triggers such as neurasthenia, psychological tension, and infections in the reproductive system (especially at the glans penis), which often allows timely correction.
Traditional Chinese medicine attributes premature ejaculation to imbalance between yin and yang. This condition has both excess and deficiency patterns: excess type is typically caused by excessive fire in the lower burner; deficiency type arises from yin deficiency with hyperactive yang or kidney deficiency with weak containment. Treatment should primarily aim to balance yin and yang. Danxi stated: "The kidney governs storage, while the liver governs release. Both organs harbor fire, connected upward to the heart. Although the control of semen lies in the kidney, its regulation is governed by the heart. When the heart’s fire stirs, the fire follows, resulting in premature ejaculation."
Ancient practitioners of sexual arts, aiming for health preservation and longevity, developed various techniques under the principle of conserving essence ("jing"), including "returning essence to nourish the brain," "absorbing yin to supplement yang," and qigong exercises such as "gong" (practice) and "cao" (exercises), along with emergency measures. While these methods contain some outdated elements, the ancient practices of mental distraction, suppressing yin, raising qi, and moving the yin organ have proven effective clinically in treating premature ejaculation, helping to train control over ejaculation and climax. Below are several traditional techniques beneficial for correcting and controlling premature ejaculation:
1. Mental Distraction
For those frequently experiencing premature ejaculation or suffering from psychological tension, the key to treatment lies in achieving mental calmness during sexual activity—what is described as "a mind as still as water"—to prevent the uncontrolled leakage of essence. The "Wang's Guide to Sexual Secrets" states: "When essence is greatly agitated, quickly tilt your head back, open your eyes wide, look around left and right, up and down, contract the lower abdomen, hold your breath, and essence will naturally cease." This method applies to individuals unable to control ejaculation but capable of vaginal penetration. Another method from "Yuling's Proven Remedies," called "Heart-Transferring Treasure Pill," advises: "Take one pill of Heavenly King Heart-Supplementing Pill, firmly grasp it in your right hand during intercourse, ensuring nails are removed so the grip is tight—otherwise, the effect fails. Store it in a silver container with solemn care, focusing your mind entirely, making it difficult for fire to escape, thus achieving lasting strength. It is even better to take Bu Zhong Yi Qi Tang combined with Cistanche and Goji berries." This method suits individuals who achieve erection rapidly but immediately reach climax, unable to control ejaculation. Through mental distraction and suggestion, the balance of yin and yang is harmonized, thereby delaying ejaculation.
2. Suppressing Yin and Raising Qi
Pressure techniques for treating premature ejaculation were already recognized by ancient Chinese sexual practitioners, though their targeted areas differ slightly from modern sexology. Jin Limeng's "Sexual Nourishment and Supplement" says: "When ejaculation is imminent, press the large pulse behind the scrotum with the middle two fingers of the left hand, exhale deeply, and gently tap your teeth ten times; then proceed to release semen, preventing it from escaping." Hong Ji's "Comprehensive Essentials of Health Preservation" also notes: "As soon as you feel ejaculation approaching, quickly withdraw the penis, press the area above the anus, and use the three fingers of your right hand to block the anal opening. Lift your breath to the dantian, swallow one breath, clear your mind, remain still, and do not move. After a moment, re-stimulate the penis, repeat the motion as before. If aroused, squat and withdraw the penis as if holding back bowel movements, raise your breath upward, and naturally ejaculation will not occur." The ancient practice of pressing the posterior scrotal artery and blocking the anal region targets a broader area than modern methods focusing solely on the base of the penis or glans, offering greater ease of control. "Calm your mind and settle your thoughts" essentially means controlling mental focus to prevent uncontrolled arousal and suppress excessive yang. The key to this technique lies in timing—applying suppression precisely at the onset of arousal.
3. Guiding Qi and Moving the Yin Organ
Qi is the fundamental driving force behind all bodily functions, capable of nourishing yin and stimulating yang. The pleasurable sensations experienced during sexual activity relate closely to spirit, essence, and qi. Controlling the rapid onset of pleasure is crucial for correcting premature ejaculation. This requires conscious mental guidance to delay ejaculation. As stated in "Ten Questions" under "The Way of Receiving Yin and Nourishing Spirit": "Treatment must be regulated: First, hang arms down, straighten the spine, scratch the buttocks; second, loosen the thighs, move the yin organ, draw in the genitalia; third, close your eyelids, refrain from listening, inhale deeply to fill the brain." Inspired by this, we often guide patients to practice morning qigong and evening stimulation of the yin organ: morning long-distance running and deep breathing; evening relaxation massage of the abdomen, back, and perineum, combined with mental exercises involving lifting the anus, lifting the testicles, and moving the yin organ. By systematically contracting and relaxing muscles in the perineum, anus, and pelvic floor, local blood circulation improves, enhancing men's ability to control ejaculation.
The effectiveness of these methods depends on consistent practice, generally requiring one to two months to show results. Though somewhat complex, they demand certain cultural literacy to maintain patience and confidence. Once the techniques are mastered and practiced consistently, premature ejaculation can be cured, and overall vitality improved, significantly enhancing the quality of sexual activity—making them highly worthwhile.
