Acupuncture Treatment for Acute Gastric Volvulus

Body acupuncture (1) Acupoints
Commonly used points: Zusanli (ST36), Jinsu (GB25), Pishu (BL20), Zhongwan (CV12).
Supplementary points: Weishu (BL21), Neiguan (PC6), Guanyuan (CV4), Sanyinjiao (SP6), Tianshu (ST25), Xiawan (CV10), Hegu (LI4).
(2) Treatment Method
Acupuncture should be performed on top of routine treatment. Prioritize common points and appropriately add supplementary points. Zhongwan and Weishu should be used together. For Zusanli, apply the "Qi Reaching the Affected Area" technique to guide needle sensation into the abdomen; other points should ideally elicit Qi arrival sensation. All points use draining method via lifting-thrusting and rotating techniques, maintaining manipulation for 20–30 minutes, with stimulation intensity increasing gradually from weak to strong. For back-shu points, electrical acupuncture may be applied using sparse-dense waves, with intensity set to the maximum tolerable level for the patient, for 30 minutes. After acupuncture, use moxibustion with moxa cone (sparrow pecking method) on abdominal points and Sanyinjiao, each for about 15 minutes, until skin becomes flushed and warmth spreads into the abdomen. Infrared radiation over the abdomen for 15 minutes may also be used. Perform acupuncture and moxibustion once or twice daily.
(3) Efficacy Evaluation
Using acupuncture as primary treatment, 89 cases were fully cured (1–7, 9).
Electroacupuncture plus intradermal injection
(1) Acupoints
Commonly used points: Shangwan (CV13) through Zhongwan (CV12), Chengman (CV14), Zusanli (ST36).
Supplementary points: Add Sanyinjiao for lower abdominal pain; add Pishu and Weishu for spleen and stomach deficiency; add Taichong and Anmian for insomnia.
(2) Treatment Method
Select all common points, adding supplementary points based on symptoms. First insert fine needles into acupoints, use balanced tonifying and draining technique with lifting-thrusting and rotating, then connect to electroacupuncture device using sparse-dense wave, frequency 14–16 times per minute, intensity adjusted to patient tolerance. After needle removal, select 1–2 points for injection of Danggui injection, 2 ml per point. Once daily, 10 sessions constitute one course.
(3) Efficacy Evaluation
Combined treatment of 42 cases resulted in 38 cures and 4 non-responses, with a total effective rate of 90.5% (10).
