Syndrome Differentiation and Treatment of Kidney Diseases in TCM

I. Key Points in Syndrome Differentiation and Treatment
Traditionally, the kidney is believed to have no exterior or real syndromes. Kidney heat results from yin deficiency, and kidney cold stems from yang deficiency. However, clinical reality shows that real syndromes do exist in the kidney—such as damp-heat accumulation, blood stasis obstruction, and stone obstruction—which are considered real syndromes or mixed deficiency-excess conditions. Kidney deficiency syndromes are generally divided into yin deficiency and yang deficiency. The overall treatment principle is “supplement the deficient, avoid harming the abundant.” For yin deficiency, spicy foods and overly bitter-cold herbs should be avoided. Sweet, moistening, kidney-nourishing formulas are preferred to supplement yin and balance yang, extinguishing false fire and returning yang to yin—“strengthening the master of water to control excessive yang.” For yang deficiency, cooling and moistening herbs and pungent-dispersing agents should be avoided. Sweet, warm, qi-invigorating substances are recommended to supplement yang and balance yin, dispersing stagnant yin and allowing yin to follow yang—“augmenting the source of fire to eliminate yin shadows.” Furthermore, based on the principle of mutual root between yin and yang, when tonifying yang, attention must be paid to yin fluids; when tonifying yin, attention must be paid to yang qi—“those skilled in tonifying yang must seek yang within yin, so yang gains assistance from yin and generates endlessly; those skilled in tonifying yin must seek yin within yang, so yin gains support from yang and its source never runs dry.” For dual yin-yang deficiency, both yin and yang should be supplemented. Kidney yin deficiency often leads to hyperactive phase fire—due to internal heat arising from yin deficiency. Treatment should primarily nourish yin, with secondary clearing of phase fire. For kidney yang deficiency, in addition to warming and tonifying kidney yang, ingredients to fill essence and marrow should be added to replenish the source of generation.
II. Classification of Syndromes
(I) Kidney Yang Deficiency
1. Kidney Qi Not Consolidated
Pathogenesis Summary: Overwork and prolonged illness lead to depletion of kidney qi, impairing its sealing and retaining functions.
Main Symptoms and Signs: Pale complexion, soreness and weakness in waist and knees, hearing loss, frequent and clear urination, residual dribbling after urination, or even urinary incontinence, or frequent nocturnal urination, men with spermatorrhea or premature ejaculation, women with clear leukorrhea, pale tongue with thin white coating, fine weak pulse.
Treatment Method: Tonify the kidney and consolidate.
Prescription Example: Shui Lu Er Xian Dan (Water and Land Two Immortals Pill), Wu Zi Yan Zong Wan (Five Seed Reproduction Pill).
2. Kidney Cannot Contain Qi
Pathogenesis Summary: Damage to kidney qi from overwork or chronic illness leads to qi deficiency, failing to return to its origin and resulting in loss of containment.
Main Symptoms and Signs: Shortness of breath and dyspnea, worse with exertion, emaciated appearance, fatigue, severe cases with exacerbated dyspnea, profuse cold sweats, cold extremities, pale face, floating pulse without deep strength.
Treatment Method: Tonify the kidney and contain qi.
Prescription Example: Ren Shen Hu Tao Tang (Ginseng and Walnut Decoction), Shen Jie San (Ginseng and Gecko Powder).
3. Kidney Yang Not Revived
Pathogenesis Summary: Weak constitution from birth, chronic illness, or overwork damaging the kidneys, leading to lower burner deficiency and diminished life gate fire.
Main Symptoms and Signs: Pale or grayish-white complexion, cold aversion and cold limbs, soreness and cold pain in waist and knees, mental exhaustion, men with impotence and infertility, women with cold uterus and infertility, or chronic diarrhea with undigested food, or early morning diarrhea.
Treatment Method: Warm and tonify kidney yang.
Prescription Example: You Gui Wan (Right Return Pill), Jin Kui Shen Qi Wan (Golden Cabinet Kidney Qi Pill).
4. Kidney Deficiency with Fluid Overflow
Pathogenesis Summary: Constitutional weakness, prolonged illness without nourishment, kidney yang deficiency, inability to warm and transform fluids, leading to fluid overflow upward or outward onto the skin.
Main Symptoms and Signs: Generalized edema, worse in lower extremities, short and infrequent urination, or coughing with upward rushing qi, copious thin phlegm, worse with exertion, pale tongue with white coating, deep fine or deep taut pulse.
Treatment Method: Warm yang and promote diuresis.
Prescription Example: Zhen Wu Tang (True Martial Decoction), Shi Pi Yin (Real Spleen Drink).
(II) Kidney Yin Deficiency
1. Kidney Yin Deficiency
Pathogenesis Summary: Excessive sexual activity, overwork, or prolonged illness depleting true yin.
Main Symptoms and Signs: Emaciated physique, dizziness and tinnitus, forgetfulness and insomnia, soreness and weakness in waist and legs, dry throat and mouth, red tongue with little coating, fine pulse.
Treatment Method: Nourish kidney yin.
Prescription Example: Liu Wei Di Huang Tang (Six Flavor Rehmannia Decoction).
2. Yin Deficiency with Fire Hyperactivity
Pathogenesis Summary: Overwork exhausting kidney yin, or post-febrile damage to body fluids, leading to internal heat from yin deficiency, with water deficiency causing fire to rise.
Main Symptoms and Signs: Flushed cheeks, red lips, tidal fever, night sweats, lumbar-sacral pain, restless insomnia, seminal emission, dry mouth and sore throat, short yellow urine, constipation, red tongue with little coating, fine rapid pulse.
Treatment Method: Nourish yin and reduce fire.
Prescription Example: Zhi Bai Di Huang Tang (Anemarrhena and Phellodendron Rehmannia Decoction).
