Acupuncture Treatment for Puerperal Infection

[Prescription] Qihai, Guanyuan, Sanyinjiao, Shenshu, Taixi, Xuehai, Geshu, Xinyu, Piyu, Shenmen, Taibai, Taichong, Baihui, Fengfu, Jinjin, Yuye, Tiantong, Quchi, Tongli, Xuanzhong, Jianyu, Hegu, Huantiao, Fengshi, Yanglingquan, Kunlun
[Procedure] Use balanced tonification and drainage at all acupoints. Treat every other day, selecting 4–6 points per session, rotating usage.
[Clinical Experience] Li XX, female, 24 years old. First visit in early August 1978. She reported giving birth in early June 1978 after a prolonged labor. On the day after delivery, she developed fever. Temperature reached 39–40°C, persisting for one week. Despite broad-spectrum antibiotics, fever did not subside. On the eighth day, she suddenly lost consciousness and experienced convulsions. Diagnosis: puerperal fever, sepsis, toxic shock. After emergency treatment, she survived but was left with complete paralysis and inability to speak. Despite treatments including cytochrome C, hormones, coenzyme A, ATP, vitamin complexes, and traditional Chinese medicine, there was no improvement. She came for consultation. Examination revealed fatigue, poor appetite, pale yellow complexion, inability to speak, drooling from the corners of the mouth, complete paralysis of limbs, tremors and twitching upon movement, inability to sit up, neck stiffness, generalized muscle laxity, defecation every few days, urinary incontinence, deep red tongue with little coating and lack of moisture, fine and sluggish pulse. After using the above acupoints in rotation, performing over 40 acupuncture sessions, she fully recovered.
[Commentary] This case involved postpartum deficiency of qi and blood, followed by invasion of external pathogens. Persistent heat scorched body fluids, leading to depletion of true yin. As a result, vital essence and blood could not nourish the meridians and tongue network, causing tongue prolapse and aphasia, limb paralysis, and overall flaccidity—known as “bi” (paralysis). Therefore, Qihai and Guanyuan were selected to invigorate qi and nourish blood, extinguishing wind. Xuehai and Shenshu were chosen to replenish qi and blood, strengthening the foundation of production and nourishing limbs and the tongue. Sanyinjiao, Shenshu, and Taixi were used to nourish kidney water and soothe liver wood, thereby calming internal wind. Shenmen and Xinyu were selected to nourish the heart and promote blood circulation, aiding speech and voice. Taichong, as the Yuan point of the Foot-Jueyin Liver Channel, was drained to pacify the liver and extinguish wind, treating paralysis and flaccidity; when supplemented, it helped control urinary leakage. Baihui, Fengfu, and Tiantong were used to regulate the yang channels of the hands, feet, and Du Meridian, extinguishing wind and treating paralysis. Quchi, Wai Guan, and Xuanzhong treated wind-induced paralysis. Jinjin, Yuye, and Tongli addressed tongue prolapse and aphasia. The entire formula thus achieved benefits of invigorating qi and blood, nourishing yin to extinguish wind, and promoting circulation in tendons and vessels.
