One, Syndrome Differentiation
I. Premature Menstruation
1. Blood Heat Type
Main Manifestations: Shortened menstrual cycle, deep red, viscous blood, heavy flow, chest tightness, irritability, red urine, red tongue with yellow coating, forceful rapid pulse.
2. Qi Deficiency Type
Main Manifestations: Heavy flow, pale color, shortened cycle, fatigue, palpitations, shortness of breath, feeling of emptiness and heaviness in lower abdomen, pale tongue with thin coating, weak pulse.
II. Delayed Menstruation
1. Blood Deficiency Type
Main Manifestations: Delayed cycle, small amount of pale blood, empty pain in lower abdomen, emaciation, sallow complexion, poor skin luster, dizziness, blurred vision, palpitations, insomnia, pale red tongue with little coating, fine weak pulse.
2. Cold in Blood Type
Main Manifestations: Delayed cycle, small amount of dark blood, pulling pain in lower abdomen, slight relief with warmth, cold limbs, thin white coating, deep slow pulse.
3. Qi Stagnation Type
Main Manifestations: Irregular cycle, small amount of dark red blood, distension and pain in lower abdomen, emotional depression, chest tightness, hiccups, distension in chest and breasts, thin white coating, wiry pulse.
III. Irregular Menstrual Cycle
1. Liver Qi Stagnation
Main Manifestations: Unpredictable cycle and flow, sticky blood, dark purple, difficult to expel, distension in chest and breasts, lower abdominal pain, depressed mood, frequent sighing, white coating, wiry pulse.
2. Kidney Deficiency
Main Manifestations: Irregular cycle, small amount of pale blood, dizziness, tinnitus, soreness and weakness in waist and knees, frequent nocturnal urination, loose stools, pale tongue with thin coating, deep weak pulse.
Three, Treatment
I. Premature Menstruation
1. Blood Heat
Prescription: Quchi (LI11), Zhongji (CV3), Xuehai (SP10), Shuiquan (KI5).
Supplementary Points:
2. Liver Fire: Xingjian (LR2);
Yin Deficiency with Internal Heat: Rangu (KI2);
Use dispersion method to clear heat and cool blood, regulate Chong and Ren vessels.
3. Qi Deficiency
Prescription: Qihai (CV6), Sanyinjiao (SP6), Zhongwan (CV12), Zusanli (ST36).
Apply tonification method.
II. Delayed Menstruation
1. Blood Deficiency and Cold in Blood
Prescription: Guanyuan (CV4), Qihai (CV6), Sanyinjiao (SP6).
Supplementary Points:
Dizziness and blurred vision: Baihui (GV20); Palpitations and insomnia: Shenmen (HT7).
For blood deficiency, apply tonification method to invigorate blood and generate qi. For cold in blood, apply balanced tonification and dispersion or moxibustion to warm meridians and dispel cold.
2. Qi Stagnation
Prescription: Tianshu (ST25), Qixue (CV14), Diji (SP8), Taichong (LR3).
Chest tightness: Neiguan (PC6); Breast and chest distension: Qimen (LR17).
Apply dispersion method.
III. Irregular Menstrual Cycle
1. Liver Qi Stagnation
Prescription: Qihai (CV6), Siman (BL17), Jian Shi (SJ5), Ligou (LR5).
Add Zhongfu (CV17) for breast distension; add Shenmen (HT7), Taichong (LR3) for emotional depression.
Apply balanced tonification and dispersion method.
2. Kidney Deficiency
Prescription: Guanyuan (CV4), Shenshu (BL23), Jiaoxin (SP8).
Soreness and weakness in waist and knees: Yaoyan (EX-B13), Yingu (KI10); Dizziness and blurred vision: Baihui (GV20), Taixi (KI3).
Apply tonification method with moxibustion.
This condition includes menstrual irregularities due to pituitary anterior lobe dysfunction or ovarian dysfunction.
