Tuina Therapy for Scapulothoracic Syndrome

Scapulothoracic syndrome, also known as "scapula-rib type syndrome," develops gradually, clinically presenting as aching and radiating pain in the scapular region. Initially, there is a heavy, stabbing sensation in the scapula, worsened by shoulder load-bearing. With prolonged duration, pain intensifies and radiates. Radiating pain may occur in the ipsilateral occiput, vertex, posterior upper arm, wrist, or hand. Severe pain may impair movement of the ipsilateral shoulder and upper limb. Thoracic wall symptoms mainly involve pain and radiation along the course of the fourth and fifth intercostal nerves. This condition commonly affects middle-aged individuals. Poor posture or habits can trigger this syndrome.
Tuina Treatment
I. Techniques: Rolling, pressing, kneading, grasping, pinching, spreading, rubbing, friction, and hot compress.
II. Points: Jianjing (GB21), Jianwaiyu (GB14), Jianzhen (SI9), Feishu (BL13), Xinyu (BL15), Zhejin (GB22), Yuanye (GB23).
III. Operational Requirements:
1. Patient sits, hands resting on knees. Practitioner stands behind, using the "grabbing" technique to embrace the scapular region—specifically the supraspinatus area, medial upper angle of the scapula, rhomboid muscles, upper trapezius, latissimus dorsi—and apply finger kneading to the 4th and 5th intercostal spaces on the affected side.
2. Patient remains seated. Practitioner stands on the affected side, one hand supporting the patient’s wrist to abduct the shoulder, the other hand using kneading technique on the upper limb—from shoulder to mid-forearm, including deltoid, biceps brachii, and forearm muscle groups.
IV. Precautions:
1. Trigger points are the most important diagnostic sign and should be targeted for treatment.
2. Radiating pain locations may be singular or combined.
3. Pain intensity varies among patients, ranging from mild discomfort to extreme agony.
4. Prone positioning is also acceptable for treatment.
5. Advise patients to strengthen self-functional exercises.
