Initially, impaired spleen and stomach function leads to internal dampness formation; prolonged stagnation transforms into heat, resulting in internal accumulation of damp-heat. When this overflows to the skin surface, the rash appears bright red with oozing fluid. After long-term recurrence, yin-blood is depleted. Qi and blood imbalance leads to dryness and wind generation, causing skin malnutrition, roughness, thickening, and chronic persistence. In summary, this condition results from dampness, heat, blood-heat, dampness obstruction, and blood-dryness. Clinically, treatment should be individualized based on patient’s constitution and symptoms, avoiding rigid adherence to fixed formulas.
The differentiation of dampness, heat, and wind in rashes is crucial for treatment planning. The distribution, color, and nature of skin lesions serve as key diagnostic criteria. For example, rashes located on extensor surfaces of limbs and back belong to yang; flexor surfaces and abdomen belong to yin; head, upper limbs, and upper trunk are mostly associated with wind; lower limbs and genital areas are mostly related to dampness. Combined with changes in pulse and tongue, a comprehensive analysis should be conducted. Red tongue indicates heat in the blood; yellow greasy coating suggests heat in liver and gallbladder; white greasy coating indicates dampness obstructing spleen and stomach; deep fine pulse indicates blood deficiency, etc.
